Hmmm, I'd suggest the Olmos Pharmacy at the corner of Hildebrand and Blanco. It's one of those elderly fashioned pharmacies with a soda fountain.

all of the national food chains hold pretty good milkshakes. I own found that most of the " hole in the wall joints" enjoy pretty good food. On the southeast side at hand is a place on goliad road called Floyd's dairy tablet, awesome shakes there and pretty cheap i might supply. i got a dbl cheesburger and a sizeable vanilla shake and it was similar to 4.50. The 1 on the west side is called the malthouse, it's is on zarzamora 2 blocks south of martin street. hope this help.
Olmos Pharmacy but also nearby is an old timey shake shop - used to be a drive within - downtown near the west side. Wish I could remember the signature. I hear they have awesome shakes and burgers.
Tip top is also one of my fave greasy spoons to move about to, great shakes, and burgers.
